ADULT PLAN - $25/month
- Two basic hygiene cleanings every six months (up to 2 per year)
- Dental X-rays
- Two dental exams per year

- One periodontal maintenance every 3-4 months (up to 4 per year)

Plan Details
- 12 month agreement required.
- Initial payment of $75 (adult) or $105 (perio) due upon enrollment. Monthly billing will begin on the fourth month.
- Patient must provide a credit card to keep on file for monthly premium payments.
- No refunds of premiums will be given if patient does not use the membership plan.
- This plan cannot be combined with any other promotional offers.
